Entry Form
The entry form can be made available for anyone to fill out by creating a left sidebar link or posting the URL to the form.
The entry output is via email per entry, or an Excel export.
Survey
The survey is published and sent to a group of users via email. A survey can only be completed once per email.
The survey has an online summary and view detail options, plus the Export.
