To run a survey or collect feedback on your Editor 2.0 website, add the website builder's Advanced Form widget to a page in the Site Editor, add a field for each question, choose who gets an email for each response, and click Republish. The responses stay with your website, under Form Responses in its Site Dashboard; they aren't saved in TeamSideline.
Applies to: Org admins whose role includes Site Editor access, such as Administrator or Content Editor · All plans, for organizations with Editor 2.0 (the CMS module) · Editor 2.0 (CMS)
Where: Content > Site Editor
Before you start
- Decide where the answers should go. A form you build in the Site Editor emails each response to the addresses you choose and keeps every response with your website. If you need the responses in TeamSideline, for example to export them from Reports > Report Exports, build an Entry Form in Content > Entry Forms and add TeamSideline's Entry Form widget to the page instead.
- Write your questions down first. Short surveys with mostly multiple-choice questions get the most answers.
Add the form to a page
- Go to Content > Site Editor and open the page where the survey should appear.
- In the left sidebar, click Add, then Elements.
- Search for Advanced Form.
- Drag the Advanced Form onto the page where you want the survey.
The form starts with a few sample fields inside a box called an inner column. You replace them with your own questions next.
Add your questions
- Click the form, then click the inner column that holds the sample fields.
- From the Form section of the widget panel, drag a field into the form for your first question. The table below shows which field suits which kind of question.
- Click the field, then click Content.
- Enter the question as the field title, and add placeholder text and answer options where the field has them.
- Turn on Required if everyone must answer the question.
- Repeat steps 2 to 5 for each question, and remove any sample fields you don't need.
| Field | Use it for |
|---|---|
| Radio button | One answer from a short list, such as a satisfaction rating |
| Checkbox | Any number of answers from a list |
| Dropdown | One answer from a long list |
| Short text | A brief written answer, such as a name or team |
| Long text | Comments and suggestions |
| Email, phone number, number, date and time | Contact details and other answers in a set format |
| File upload | A document or photo from the person answering |
| Opt-in field | Permission to contact the person later |
For example, a season survey might ask "How satisfied are you with registration?" with the radio buttons Very Satisfied, Satisfied, Neutral and Unsatisfied, followed by "What can we improve?" as a long text field.
Finish and publish the form
- Click the form's submit button, click Manage Button and enter the button text, such as Send Feedback.
- Click the form, click Manage Form and open the Manage Steps tab. Click Success to edit the thank-you message people see after they submit, and Error to edit the message they see if a submission fails.
- Click the form and open Settings. Enter a Form Name you'll recognize; the responses are listed under this name.
- In Notify by email to, enter each address that should get the responses. Each new address gets a confirmation email first, and responses reach it only after someone clicks the link in that email.
- Click Preview to check the form.
- Click Republish to put it on your live website.
- Submit a test response on your live website and check that the email arrives.
Longer surveys
- Add a text widget above each group of questions as a heading, such as Registration, Communication, Scheduling and Website.
- To split the survey into steps, click the form, click Manage Form, open the Content tab and click + Add Page. The three-dot icon next to a page renames, duplicates or deletes it; a duplicate keeps the page's layout.
What you'll see
After you republish, the survey appears on the page. When someone submits it, they see your success message, each address in Notify by email to gets the response by email, and the response is listed under Form Responses in your website's Site Dashboard. To view, export or forward responses, see the article CMS: View, export and get email notifications for form and survey responses.
If it doesn't work
- The survey isn't on your live website. Click Republish. Visitors see Site Editor changes only after you republish.
- No one gets the response emails. Check that each recipient clicked the link in the confirmation email, that their address is in this form's Notify by email to box (each form has its own recipients), and their spam or junk folder.
- The responses aren't in TeamSideline's reports. That's expected: forms built in the Site Editor keep their responses with your website. Use an Entry Form and the Entry Form widget when you need responses in TeamSideline.
- The editor shows an Upgrade prompt. Some website-builder tools aren't included with TeamSideline websites. Contact TeamSideline Support instead of using the prompt.
- An older survey link opens your home page. It's a survey from TeamSideline's Communications > Surveys tool. Those surveys open on pages of your legacy website, which send visitors to your new home page once your Editor 2.0 website is live. Rebuild the survey as a form on your website.
Good to know
- Your Site Editor may also offer a ready-made Survey section: click Add Section, choose Designed, open the TeamSideline category and choose Survey. It's a website form like the one above: edit its questions, set who gets its responses, and republish.
- Responses aren't linked to families' TeamSideline accounts. Ask for a name or email address in the form if you need to know who answered.
- Keep each survey to one topic and make only the essential questions required. Put it on a busy page, such as your home page or a registration page, or send families the page's link in an email.
- For branching questions or detailed analytics, use a dedicated survey service and link a button on your page to it.
