Responses to the forms you build in the Site Editor, such as surveys, feedback forms and contact forms, stay with your website. Each response is emailed to the addresses set on that form, and every response is listed under Form Responses in your website's Site Dashboard, where you can export them to a CSV file. They aren't saved in TeamSideline, so they don't appear in Reports > Report Exports.
Applies to: Org admins whose role includes Site Editor access, such as Administrator or Content Editor · All plans, for organizations with Editor 2.0 (the CMS module) · Editor 2.0 (CMS)
Where: Content > Site Editor, then Site Dashboard > Form Responses
Choose who gets an email for each response
Each form has its own recipients, so changing one form doesn't change your other forms.

- Click the form and open Settings.
- Enter each address in Notify by email to.
- Click Republish.
Contact Form

- Go to Content > Site Editor.
- Open the Site Dashboard: click the menu icon (three lines) at the far left of the top bar, or click the information icon (i) and choose Take Me to Site Dashboard.
- Choose Form Responses.
- Select the form by its form name to see its responses.
Export responses to a spreadsheet
- On Form Responses, select the form.
- Click Export to CSV.
- Open the downloaded file in Excel, Google Sheets or another spreadsheet program.
Send responses to other tools
An Advanced Form can also pass each response to another service. Click the form, open Settings and use Integrations. Depending on your website, the choices may include Google Sheets, Mailchimp, Zapier, Google Drive and Slack. If an integration shows an Upgrade prompt, contact TeamSideline Support instead of using it.
What you'll see
Each response arrives as an email with the visitor's answers. Form Responses lists every response to the form you select, including any whose email didn't reach you.
Where each kind of website form sends its responses
| Form | Where responses are kept | Who gets an email |
|---|---|---|
| Advanced Form, Contact Form or Survey section (built in the Site Editor) | Your website's Site Dashboard, under Form Responses | The addresses set on that form, plus your website's main account email |
| TeamSideline's Entry Form widget | TeamSideline: View Responses on Content > Entry Forms, and Reports > Report Exports > Entry Form Responses | Email Address 1 to 5 on the Entry Form, plus the person who submitted it when the form has an Email field |
| TeamSideline's Contact Us widget | TeamSideline: Communications > Contact Us Tickets | The addresses set for the subject the visitor picks, otherwise the Email Address on Communications > Configurations > Contact Us; the visitor gets a confirmation email |
If it doesn't work
- A recipient gets no response emails. They haven't clicked the confirmation link yet, their address isn't on this form, or the emails are going to spam. Check the form's settings, republish, and submit a test response.
- Another address gets a copy of every response. Your website's main account email address also receives each submission, like a Cc, whatever the form's own recipients. If that address needs to change, contact TeamSideline Support.
- The responses aren't in Reports > Report Exports. That's expected. Only TeamSideline's Entry Form widget saves responses in TeamSideline; export website form responses from Form Responses instead.
- You're looking for answers to a survey you emailed from Communications > Surveys. Those results are in TeamSideline's Surveys tool, not in Form Responses.
Good to know
- Give each form a clear Form Name in its Settings so its responses are easy to find.
- Website form responses aren't linked to families' TeamSideline accounts, so they can't be matched to players or added to TeamSideline reports. Export them when you need a record.
- Changes to a form's recipients reach your live website when you republish.
- To build a survey or feedback form, see the article CMS: Create a survey or feedback form on your website.
